Cardiff University failed to control exposure to animal allergens for over 15 years, despite guidance being available since 1989. Two employees developed lifelong respiratory conditions — one was unable to continue working.

Firm fined £140,000 after apprentice injured by unguarded guillotine in training workshop

MTL Advanced Ltd was prosecuted after a 17-year-old apprentice's thumb was crushed by a metal cutting guillotine during training. HSE found a large gap allowing access to dangerous parts — a risk the company failed to identify even after the incident.

Ecclesiastical Steeplejacks Ltd was prosecuted after an employee died following a fall from St Nicholas' Church steeple, Birmingham. Investigators found inadequate fall prevention and a failure to plan and supervise work at height.

Council fined after worker seriously injured when mower overturned in churchyard

Bassetlaw District Council was prosecuted after an employee was seriously injured when a ride-on mower overturned at a churchyard in Nottinghamshire, causing the worker to fall more than two metres onto a public road. No adequate risk assessment was in place.
